Dr. Gruizenga grew up in the Kalamazoo area attending Western Michigan University, where she graduated Magna Cum Laude with a degree in Biomedical Sciences.
In 1990, she graduated from Indiana University School of Optometry and received the John P. Davy Memorial Award. Dr. Gruizenga began practicing at Hiemstra Optical later that year.

Anytime the site is asking for your Patient ID, it is referring to your account number. On the top left corner of your statement you will see the practice name and logo as well as Patient Name and Patient ID. This Patient ID is your account number and is what you will want to use in the Patient ID field on InstaMed.

The plans can be set up with either a credit card or checking account. The site will automatically calculate the payments for the frequency and duration you choose.
